Q: How could I understand that lantus works 24 hours in my body not less?

A: It is because the insulin in lantus is slightly different from the insulin in humalog. Both are recombinant human insulin analogs but lantus (or glargine) has a few amino acids that are different. This difference makes it insoluble in neutral pH (~7.0, blood is pH ~7.4) and soluble in acidic pH (the solution in which the glargine is dissolved is at about pH 4). When the glargine is injected into the blood, the neutral pH causes the glargine to form little clusters which release a little bit of glargine insulin at a time. It results in a steady release of glargine insulin over 24 hours without a noticeable peak.

Q: What happens if you use the same needle to inject Humalog and Lantus but at different injection sites?


A: You could easily contaminate both bottles causing the wrong dosage of the correct insulin. Also a needle dulls very quickly and hurts more if you use the same needle twice. In an emergency it is better to have the insulin instead of going without but I do not suggest making a habit of using the same needle more than once.

I am a case of IDDM i intected 55 unit mixtard 30/70 two times per day. Now I shiftted to Lantus and HUMALOG. I inject 28 unit Lantus AT BED TIME AND APROX 5 UNIT HUMALOG AFTER BREAKFAST 8-10 UNIT HUMALOG AFTER LUNCH AND DINNER .Do I USE THE RIGHT DOSE? MY BS IS IN normal range.Have you any recomendation for me?

A: The best advice I ever got about this was that your blood sugar in the morning should be the same as it was before you went to bed, because this means that your background dose (lantus) is right. You should ask your dr about carb counting or dafne courses if you have type 1 diabetes as these can help you to get your control better and mean that you have heaps more freedom about what and when to eat. But if your blood sugars are ok then that's a pretty good sign your dose is right for you. I now use a pump but you're on almost exactly the same as what I took when I was on lantus/humalog. Remember everyone needs different amounts of insulin depending on their metabolism, diet, level of exercise etc. - it's really a case of trial and error to find out what's right for you, but if you're blood sugars are good and you're not feeling ill (esp. if you're not feeling like you've been hit by a bus in the mornings as this means you could be hypo during the night and might need to reduce your lantus dose) then don't worry - you're doing better than most! i'm 20 and have had type one diabetes for 10 years. I'm wondering how much insulin would cost with out insurance because I might possibly be without it soon! So i wanted to know if anybody knew how much lantus and humalog would cost without ins. I have perscriptions for it right now..but my ins. is done at the end of March...

Q: How can I tell if I've taken too much Lantus Insulin?
I normally take 25 units in the morning. This morning, I took it 2 hours off the usual time because I had a nosebleed (dry weather) and had to take care of the nosebleed first. I don't remember taking it when I first woke up, but I did have the needles, not the insulin out. My log doesn't show that I took any when I first work up. So I don't remember taking it earlier, and if I did, I didn't make a log. Is there any way I can, with my glucometer, check to see if I took a double dose or not? "And it was ok to take care of the nosebleed first, but do the same with the log. " I'm not sure what this means. You must never have had the experience of taking insulin and forgetting the log, or jotting the log first and getting distracted and not takingthe insulin. Because there are two steps involved, if something happens between one and the other, even the log doesn't do any good.

A: If you have been keeping a good log, and can see what your average glucose has been for the same time of the day, for the past , let's say, week, and you are significantly below that (maybe 10-20 points) you might have taken a double dose. Are you below 70? Then you might have taken a double dose. If you drop 50 points in an hour, that may be a sign too. Are you familiar with what too low of blood sugar feels like? If you start to get dizzy, or fuzzy-headed, you may want to eat some peanut butter or drink orange juice (not much - maybe 4 oz) to hold it off. I only took oral meds, so I don't have personal exoerience - but I was hypoglycemic (too low of blood sugar) before I was diabetic, and remember what that felt like. If you are really worried, call an ER or your doctor.

Q: I use Lantus Optiset Insulin Injection, How important is it to use new needle everytime?

A: Even though you are not sharing the needle with someone else, you do run a higher risk of developing an infection if you continually reuse your needles. Anytime you stick a needle beneath your skin, it picks up biological material (blood, skin cells, etc.). When that needle is removed from the body and exposed to air, germs will automatically be attracted to those materials on the needle, causing the needle to become one giant feeding ground for germs. Therefore, probably not a good idea to reuse your insulin injection needles. Sorry! Talk to your doctor for help in finding out where to get new needles or refer to your pharmacist. Q: Has anyone taken lantus, humalog insulin and then been prescribed metformin hydrochloride tablets?
This is usually for type 2 diabetics, I was wondering how the combination worked together?

A: Hi, I've been on Metformin of one formulation or another for 10 years. Then about 4 years ago the doc added Lantus, and just recently we added the humalog. I wish to get rid of the Metformin HCL, but don't think it would work well. It gives me nasty gas problem no matter what I do, but it also helps keep the liver from dumping stored glucose on the system. As for the other way round, on one forum I read of a lady who has recently become insulin resistant and had Metformin added to her type 1 insulin regimen. that is called by some Endos "double diabetes". And yes, she is able to control her glucose levels much better that way.

A: Glucophage (metformin) is an oral medication taken to decrease blood sugar levels. It works by decreasing the production of glucose by the liver and by improving the sensitivity of a person's natural insulin. Glucophage is the most prescribed oral diabetic agent in Type 2 diabetics. It is safe with minimal side effects. Some patients will lose weight with glucophage which is an added benefit. Lantus is a type of insulin known as basal insulin. It is injected below the skin once a day. It works by mimicking the bodies normal production of insulin and is quite effective. Glucophage and Lantus may be used in combination for optimum glucose control. For your other question, morphine is a potent drug which is effective for patients with pain. It is addictive and if it is not needed to treat pain then the person taking it should seek help for drug dependence. If too much is taken it can lead to respiratory depression (stop breathing).

A: Do you have any way to get more?? Ask the pharmacist to loan you enough until you can get the doctor to refill it.If he wont and you don't want to go to the E.R. just be real careful and NO SUGAR! stay clear of any breads or carbs, as they turn to sugar, eat a lot of veggies and fruit but if you start getting to dizzy or feeling real bad you will have to go to ER. no way out of it yo could go into a diabetic come...How high does your sugar run > If it tends to get over 299-300 then you will need to be extra careful but anything higher then 300 you must go in...

Q: Lantus at night or in the morning?
I am new with Lantus, but my doctor told me to start taking the Lantus at night. I slowly raised it to 20 units and now I am having lows at around 6:00 or 7:00. The rest of the day my sugar levels seem to be high. Will it make a difference if I take Lantus in the morning, or do I have to raise my usual Novorapid dose before meals?

A: The lows could be caused by too much short acting insulin at your dinner, or bedtime snack. I assume you mean you are getting low at 6 or 7 AM, so just lower your novorapid dose at dinner or snack, take the 20 lantus, and see how it goes. If your sugars are still high at meals, then increase your novorapid. Make one change at a time though, so that you can know what is working and what isnt. :)

A: As you know, Lantus is a slow release acting insulin specifically designed for a 24 hour period., so it will act slowly but steadily, if your looking for immediate results then you should be on a fast acting insulin taken just before meals, depending on the amount of carbs. your eating. Lantus and other slow releasing forms of insulin should betaken by diabetics who have A1c's a bit higher than the recommended value....approx.6.5%, if your A1c's are above 7% than a faster acting form of insulin is required but you need to discuss this with your physician.

Q: Does Lantus really expire after 28 days?
I have started my cat on Lantus and the Pharmacy says it will need to be discarded in 28 days. I have found quite a few websites stating that when using Lantus with cats that the insulin will last up to 6 months opened and kept in the refrigerator. I hope this is the case because this insulin is very expensive and I know I will have alot leftover at the end of 28 days. Any help from anyone will be greatly appreciated.

A: I think that the Pharmacy is referring to insulin that has not been refrigerated. Most people open a new insulin and keep it out of the fridge (because injecting insulin straight from the fridge is painful) and dispose of it once it has finished or once 28 days have passed (whatever comes first). So, if it is kept in the fridge it will probably last for 3-6 months. Just check before each injection that it has not become cloudy or developed granules, if it has throw it out. And don't use it if it has passed its expiration date. Hope that helps!

Q: Do you take Lantus in the evening or morning?
I have to take mine in the morning. For some reason I would get really low in the middle of the night if I took it at bedtime. Does this happen to you? I take 10 units of Lantus in the a.m. Along with Humalog as needed before meals. But I think my weight has a lot to do with it. I am 5'8" and 115lbs. Very thin. Thank You Grandma, that brought tears to my eyes.
