I take brazilian jiu-jitsu. I have gotten numerous fungus infections from the sport. A real down side. I am 39 and I have always had super thick hair. A: Very good for susceptible fungi. Care must be taken to avoid drug interactions with which can be severe if careless. Itraconazole is the generic name for Sporanox, one of the azole (a triazole, specifically) antifungals Indications: [1] Systemic mycosis (fungi widespread through the body): 200mg orally once daily with food (up to maximum of 400mg/day if unsatisfactory clinical response with lower dose). Doses >200mg are given in 2 divided doses. [2] Onychomycosis(fungal nail bed infections): 200mg orally twice daily for 1 week each month for 2 months (fingernails); x 3-4 months (toenails). [3] Oropharyngeal candidiasis("thrush" in the mouth) 200mg (20ml)-oral solution-swish vigorously then swallow once daily x 1-2 weeks. A: There shouldn't be any damage done, the pregnancy was in the very early stages - in those early weeks, before you're even aware you're pregnant, the building blocks are being laid for development (cell division, etc) but the brain, CNS, etc aren't actually growing yet. Try not to worry about it. There are loads of medications not recommended in pregnancy but they can still be described with no negative effects on the baby. I had back spasms earlier in my pregnancy and was given a shot of morphine for the pain. A few weeks ago I fell in the bath and hurt my ribs and was prescribed codiene. Neither drug is recommended during pregnancy, but they are fine if given under the right circumstances and in the right dose. Now that you're pregnant, just let your doctor know each time you're prescribed something so they can make sure it's the right medication at the right dose.

I have 8 puppies who all have ringworm. It started with a stray cat I saved who didn't have signs but was a carrier. I've been dealing with this for over 2 months. Some of the pups have it worse than others but it's not going away no matter what I try. I've had them to 3 different vets and have been given shampoos, ointments, oral medications, etc. I bought FungRX off the internet, which hasn't worked as of yet (2 weeks). Also, natural Fungas creams and even Tinactin from the drug store. Right now the puppies are on an oral perscription medication called Sporanox ($200.00 a bottle, lasts 2 weeks) twice a day, along with the oily type ointment called FungRX that is put on the ringworm also twice daily. I am bleaching everything surrounding the pups as well. This has been so frustrating, costly, and time consuming. I feel so bad for these little guys but I can't give up. A: Lisa, Please ask your vet and do research about Selenium Dioxide which is the active ingredient in extra strength Selsun Blue. It is not intended to be used on large portions of the body at once. Also, apple cider vinegar in a watered down solution. And possibly clipping fur to allow easier access to air and light. I believe fungi don't do well with either. Important! I'm basing this on my experience with my dog who had suffered with skin allergies for several years. I went the vet route and did shots and had him on antibiotics and spent a lot of money doing what they told me to. Also, he had to wear one of those stupid cones on his head all the time. I finally got the notion that allergies just started the problem. He had symptoms of a bacterial infection and I felt that we just needed to break that cycle. All I can tell you is, I was desperate after so many years and that this worked. I don't know if it was the hair cut, the Selsun Blue or the Vinegar or all three. Please be careful. After 12 years, I had just to put that guy to sleep and I would hate it anything I told you harmed your dogs. Again, please research it yourself and run it by your vet.

I've been diagnosed with pityrosporum follicultis. I have the white and red bumps all over my neck,chest and back area...The doctor prescribed Sporanox(itraconazole) and Nizoral shampoo. I've been on the pills for 11 days, 400mg/day. But I haven't seen much improvement. What should I do? Please help!

A: Lily, Malassezia folliculitis (previously called ‘pityrosporum folliculitis’) is due to proliferation of a yeast, called malassezia, within the hair follicles. The causes of malassezia folliculitis are not fully understood but the following are believed to be important - external factors include - the yeast tends to overgrow in hot, humid, sweaty environments. Sweating is encouraged by wearing occlusive clothing. Sunscreens and greasy emollients may occlude the follicles. The host factors include - Oily skin (provoked by hormonal influences). Obesity. Pregnancy. Stress or fatigue. Systemic illness, including - Diabetes mellitus. Immune deficiency. Medications, such as - Broad spectrum oral antibiotics (often prescribed for acne), which suppress skin bacteria allowing yeasts to proliferate. Oral steroids such as prednisone (steroid acne). Oral contraceptive pill. Both topical and oral antifungals are effective agents in the treatment of Pityrosporum folliculitis (PF). Oral antifungals have the advantage of dramatic, immediate clearing of the lesions and are the most effective treatment. Patients have been successfully treated with oral pulse itraconazole and weekly fluconazole. A course of oral ketoconazole and topical ketoconazole shampoo (Nizoral) is currently the recommended treatment. Oral medication should be discontinued when the lesions resolve. Because relapse almost always occurs when treatment is withdrawn, topical ketoconazole is indefinitely continued after successful initial treatment with oral medication. Other topicals that are used to treat Pityrosporum folliculitis are ciclopirox olamine cream, econazole cream, alcohol and salicylic acid solution (with or without benzoic acid 5%), propylene glycol 50% in water, and selenium sulfide shampoo. In cases associated with antibiotic use, discontinuing the antibiotic may be helpful. Retinoids, which are used for comedones in acne, have no effect because no comedones are present in Pityrosporum folliculitis. Tetracycline does not help in Pityrosporum folliculitis, and if prescribed, it may exacerbate the condition by further destroying the normal bacterial skin flora and allowing further spread of Malassezia yeasts. Specific treatment can be divided into - topical treatment = Antidandruff shampoo as a cleanser. Topical antifungal agents, especially ketoconazole or ciclopirox creams or econazole foaming solution. Oral treatment = Azole antifungal agents including - Ketoconazole and Itraconazole (which are what you have been prescribed). Isotretinoin to reduce seborrhoea. It would seem that your doctor has your treatment ‘in hand’, so to speak and you are receiving what could be considered as the ‘normal’ and recommended medications. Bear in mind that these conditions are not cured or caused to vanish over night. As mentioned above they also have a tendency to reappear. You would be advised to discuss your concerns with your doctor and determine if there is another way forward if the present regimen is not showing any improvement.
